Core Health Group Corp. provides in home nursing and personal support services to medically complex children across ten licensed Georgia counties. Our families trust us with tracheostomies, ventilators, feeding tubes and seizure disorders, so our hiring file has to be complete before anyone sets foot in a home.

This packet is an application, not an offer of employment. Submitting it begins the screening process only. As assignments become available, candidates are reviewed against the needs of the case. If selected to move forward, we will contact you to schedule an interview. This application stays active for 45 days.

Fingerprinting and the criminal background check: we set up your appointment. You pay the fee at the appointment. Do not arrange or pay a third party on your own.

• Current CPR/BLS and Basic First Aid certification. RNs and LPNs: upload your BLS (Basic Life Support) card. Must be an in person, hands on skills course. Online only certification is not accepted.
• Tuberculosis screening results dated within the last twelve months. TB risk assessment plus test results, or a negative two step PPD or IGRA.
• Nursing license, CNA certificate, or evidence of approved competency examination. RN and LPN applicants: current unencumbered Georgia license. Personal support applicants: Georgia CNA registry certificate or documented competency evaluation.
• Resume or written summary of experience caring for medically fragile or medically complex patients. Required by GAPP for every nurse and nursing support staff member before assignment.

• Valid government issued driver's license or state ID
• Social Security card

Section 7 is not in this packet. The background check and fingerprint authorization collects your Social Security number and date of birth, so it is never completed through a web form or sent by ordinary email. The office issues it to you directly once your application is reviewed, and you return it by secure fax, by mail, or in person. Your application is complete without it.
