Skilled nursing services may be covered under Medicaid (including GAPP for pediatric cases) and private pay. We conduct a free eligibility assessment before services begin to confirm your coverage.
RNs (Registered Nurses) perform comprehensive assessments, develop care plans, and manage complex procedures such as IV therapy and ventilator care. LPNs (Licensed Practical Nurses) implement established care plans, administer medications, and provide hands-on clinical support under RN supervision.
GAPP (Georgia Pediatric Program) is a Medicaid waiver for medically fragile children. As an approved GAPP provider, Core Health Group can deliver skilled nursing care — including ventilator and tracheostomy management — at no out-of-pocket cost to eligible families.
After a physician order and initial assessment are completed, skilled nursing services can typically begin within 24–72 hours. For urgent GAPP cases we work to expedite the process as quickly as possible.
