Georgia families can change their GAPP home health agency at any time, for any reason. Your existing authorization follows your child to the new provider. Core Health Group Corp. coordinates every step so care continues without interruption.

No Re-Authorization Required

Your child's existing GAPP authorization transfers to the new provider. There is no need to restart the clinical review or submit new documentation to Alliant Health Solutions.

No Gap in Care

Core Health Group Corp. coordinates directly with your current provider, your child's physician, and the Medicaid case manager to ensure continuity of care during the transition. Our goal is zero gap.

Your Choice is Protected

Under Georgia Medicaid rules, families have the right to select any qualified GAPP provider at any time. No provider may penalize a family or obstruct a transfer.

Call or submit an inquiry. Our team will explain the transfer process, answer questions, and confirm that we can accept your child's authorization.

We Coordinate With Your Current Provider

We notify your current provider of the upcoming transfer and establish a transition timeline. We request records and care documentation to ensure continuity.

Notify Your Medicaid Case Manager

We assist your family in notifying the assigned Medicaid case manager or care coordinator about the provider change. In many cases we handle this communication on your behalf.

Care Begins With CHG

Once the transfer is complete, your CHG nurse or caregiver begins services under the existing authorization. No interruption to approved hours or services.

Will my child lose any authorized hours when switching providers?

No. Your authorization covers specific services and hours, not a specific provider. Those authorized hours transfer with your child.

Do I need a reason to switch providers?

No. Georgia Medicaid grants families the right to choose their GAPP provider without requiring any justification. You may switch at any time.

How long does a provider transfer take?

Most transitions are completed in 1–2 weeks. Core Health Group Corp. coordinates all logistics and works to minimize the time between your decision and the start of services.

What if my current provider won't cooperate with the transfer?

Contact us — we can assist. Georgia Medicaid policy requires providers to cooperate with authorized transfers. If a provider is obstructing the process, we can help your family escalate to the appropriate Medicaid contacts.

Can I switch back to my previous provider if needed?

Yes. Provider choice is ongoing. You may choose any qualified GAPP provider at any time, including returning to a prior provider.
